Medical Forms

The Department of Intercollegiate Athletics welcomes you as a sports camp participant. Every camp carries some degree of risk to the participant because of the physical nature of the activities. The UT Sports Medicine Staff provides specialized services, care and supervision to safeguard your health and well being.


Medical Care/Insurance

Campers' parent or legal guardian must complete the required medical and transportation forms prior to the start of camp. Note: All forms can now be completed on-line via DocuSign.

A current physical is no longer required to participate in camp;
however, the camper’s parent or legal guardian must sign off on the "Pre-Activity Clerance Examination & Immunization Record" section of the medical forms in order to participate in camp.

Each camper must be covered by his/her parents/legal guardian’s insurance before participating. Medical aid is provided on site by a licensed athletics trainer. Campers’ parents/guardians are billed directly for all medical services rendered by urgent care facilities, local hospital, or other medical care facilities outside of UT Sports Medicine and not covered by their insurance.

Campers WILL NOT be permitted to participate in any activities until all forms are completed and are on file with the camp supervisors.


Forms
Once you complete your on-line registration, you will receive an email from DocuSign that will provide you with a link to our release forms. To finalize the forms you are required to complete, sign and submit them 2 business days prior to the start of camp, or your child will be ineligible to attend any Texas Football Camp.


The email link you receive will transfer you to docusign.net which requires you to approve your electronic signature as your official consent on the subsequent forms. Once your consent is acquired the DocuSign website will guide you through finalizing and signing the forms.

If you do not receive the release forms once you complete the on-line registration from dse@docusign.net, please check your junk email folder.

DocuSign (http://docusign.com/) is trusted and has been approved by The University of Texas at Austin for its release forms. If you have any questions or concerns regarding this matter, please feel free to contact our office.


University policy requires that we have all forms completed and signed by a parent and/or guardian before we can release a camper to participate. The easiest way to complete these forms is via the DocuSign link HERE
